Vitreous Hemorrhage

Cause: Bleeding into the vitreous due to diabetes, trauma, or retinal tear.

When vitrectomy is needed:

Dense or non-clearing hemorrhage (>1–3 months)
Recurrent bleeds or when visual obscuration persists

Procedure:

Anesthesia: Local (retrobulbar/peribulbar) or general, based on patient and surgeon preference.
Three small incisions (ports) are made in the pars plana (white of the eye, 3.5–4 mm behind the cornea).
Infusion cannula is inserted to maintain intraocular pressure with a balanced salt solution.
Endoilluminator is inserted to light up the inside of the eye.
Vitrector is inserted to remove the blood-filled vitreous gel.
Posterior vitreous detachment (PVD) is induced if not already present.
Search for bleeding source:
- If due to diabetic retinopathy, pan-retinal photocoagulation (laser treatment) is applied to the peripheral retina.
Fluid-air exchange may be done to improve clarity.
Gas bubble (e.g., SF₆ or C₃F₈) may be used if there’s a retinal break.
Ports are removed, self-sealing wounds checked or sutured if needed.

Goal: Restore clear vision, allow for treatment of underlying problem.

Success Rate: ~60–70% restore good vision if retina intact.

Risks: Rebleeding, retinal tear, cataract.

Side Effects: Temporarily blurry vision, eye discomfort.
