Debilitating Floaters

Cause: Dense floaters interfering with quality of life.

When vitrectomy is considered:

- Dense, visually disabling floaters not responsive to laser vitreolysis
- After careful risk–benefit evaluation (due to surgical risks like cataract, retinal detachment)

Procedure:

Local or general anesthesia, depending on patient preference and age.
Three tiny ports placed in the sclera (pars plana).
Infusion line inserted to keep the eye pressurized.
Vitrector tool used to:
- Remove the core vitreous (central gel)
- Remove any visible floaters
No membrane peeling, laser, or gas bubble is usually needed unless an unexpected finding is seen.
Peripheral shaving may be done to reduce the risk of later retinal tears.
Wound sites checked; most are self-sealing (no stitches).

Goal: Clear floaters and restore clarity.

Success Rate: ~95% satisfaction for severe cases.

Risks: Retinal tear, cataract, infection.

Side Effects: Light sensitivity, mild inflammation.
