Epiretinal Membrane
Cause: Scar-like membrane grows on the macula, causing visual distortion or blurriness.
When vitrectomy is needed:
- Moderate to severe visual impairment
- Failure of conservative management
- Moderate to severe visual impairment
Procedure:
- Anesthesia: Local, typically with monitored sedation.
- Three-port vitrectomy initiated as in standard setup.
- Core vitreous removal with vitrector.
- Posterior hyaloid separation is induced to fully access the macula.
- Dye (e.g., Brilliant Blue) may be injected to stain the membrane.
- ERM removal: A fine-tipped forceps is used to grasp and peel off the membrane from the retina.
- ILM peeling may also be done to reduce recurrence.
- Fluid-air exchange is optional; tamponade is rarely needed unless complications arise.
- Ports removed, eye may be patched temporarily.
Goal: Flatten the retina and reduce distortion.
Success Rate: ~80–90% improve or stabilize.
Risks: Retinal tear, cataract, bleeding.
Side Effects: Vision may improve slowly over months.
