Diabetic Retinopathy Vitrectomy

Proliferative Diabetic Retinopathy

Cause: Abnormal blood vessel growth and scarring from diabetes, which may bleed or pull on the retina.

When vitrectomy is needed:

    • Non-clearing vitreous hemorrhage

    • Tractional retinal detachment threatening the macula

    • Fibrovascular proliferation causing traction

    • Often preceded by intravitreal anti-VEGF

Procedure:

  1. Anesthesia: Often general due to complexity.

  2. Three-port pars plana vitrectomy setup.

  3. Core vitreous and hemorrhage removed with vitrector.

  4. Induce posterior vitreous detachment (PVD) if not already present.

  5. Dissect fibrovascular membranes that are pulling on the retina (often tightly adhered).

    • May require scissors or segmentation tools.

  6. Diathermy (cauterization) may be used to stop active bleeding.

  7. Pan-retinal photocoagulation (laser) is applied to reduce ischemia and neovascular drive.

  8. Tamponade:

    • Gas if there’s retinal traction.

    • Oil if complex or reoperation likely.

  9. Ports removed, wounds examined for leaks or bleeding.

Goal: Stop bleeding, relieve pulling on retina, prevent detachment.

Success Rate: Good if done early (anatomical success ~80–90%).

Risks: Recurrent bleeding, scar tissue growth, glaucoma.

Side Effects: Cataract, visual recovery varies.
