Endophthalmitis Vitrectomy
Endophthalmitis (Infection inside the eye)
Cause: Severe eye infection from surgery, injury, or internal infection.
When vitrectomy is needed:
- Severe or worsening infection despite antibiotics
- To remove infected vitreous and improve antibiotic penetration
- Emergency vitrectomy in vision-threatening cases
- Severe or worsening infection despite antibiotics
Procedure:
- Anesthesia: Usually local, but general may be used in severe cases.
- Three small incisions are made in the white part of the eye (pars plana).
- Infusion line is placed to keep the eye pressurized with a sterile fluid.
- Vitrector tool is inserted to remove the infected vitreous gel.
- Samples of the removed fluid are sent to the lab to identify the infection.
- Irrigation of the eye with balanced salt solution or antibiotics.
- Intravitreal injection of antibiotics and/or antifungal medication.
- Wound closure if needed (usually sutureless).
Goal: Clear infection, save eye structure and vision.
Success Rate: Depends on timing — early treatment gives the best outcome.
Risks: Severe vision loss or loss of eye if infection is advanced.
Side Effects: Eye pain, blurry vision, inflammation.
